CONSENT TO TELEHEALTH SERVICES

European Union and United Kingdom

By providing your consent, you authorize Sword and the Professionals (each as defined in Sword's Terms and Conditions) to provide you, or the child on whose behalf you are acting, with treatment and services through the use of telehealth services, which involves the provision of physical therapy or other wellness services, or, where the Services include AI Pediatrics, pediatric healthcare services, by Professionals in a different physical location than yours through the Services. There are risks associated with providing these services remotely, including potential delays in receiving treatment and disruptions caused by technological failures. You understand that (1) you have no obligation to receive telehealth services and may instead seek in-person services, (2) your telehealth service provider will not be able to examine you, or the child, as applicable, in person and, therefore, may be limited in their ability to assess your, or the child's, condition, (3) you may seek assistance or file a complaint with the competent professional body for the relevant Professional's discipline at any time and (4) you may not use the telehealth service to receive emergency care or any type of care that requires in-person treatment. Contact emergency services or your, or the child's, doctor if you have a medical emergency. Any costs and charges associated with receiving care by telehealth through the Services are as otherwise described at enrollment. Consultations provided through the Services are not audio or video-recorded; however, content you submit may be stored and/or accessed for the provision of the Services, as described in the Privacy Statement. You agree that (a) you have had the opportunity to ask questions about the healthcare services provided remotely, (b) you are fully informed about the healthcare services provided remotely and (c) that all terms of this document apply to those services.

Where you are consenting on behalf of a child in connection with AI Pediatrics, you confirm that you hold parental responsibility, guardianship, or equivalent legal authority over the child, or have been given authority to do so by such a person, and that this consent is given by you in that capacity and not by the child.

You have the right to refuse or withdraw your consent to the use of telehealth services in the course of your, or the child's, care at any time, by emailing help@swordhealth.com, without affecting your right to future services. Withdrawing your consent to the use of telehealth services means that you, or the child, as applicable, will not be able to use the Services..
